School and Sports Physical Exams

School Entrance Health Form

School Entrance Health Form, MCH 213F, completed by a licensed physician, nurse practitioner or physician’s assistant, must be presented at the time of enrollment of your child. The three-part form includes Part I-Health Information Form, Part II Certification of Immunization, and Part III-Comprehensive Physical Examination Report. The report must indicate that the child has received a comprehensive physical examination performed within the twelve months prior to the date the child first enters a public kindergarten or elementary school (K-5). The physical examination report must include “Recommendations to School” and other information as required by school division policy.

School Sport Physical

If your child wants to participate in sports at his / her school in Virginia, a Sports Physical examination must be done by a licensed physician, Nurse Practitioner or a Physician’s Assistant. This is required by Virginia Law.
School Sport Physical Exams Involve Checking For:

  • Height & Weight
  • Blood pressure and pulse
  • Heart murmurs (abnormal sounds)
  • Vision
  • Breathing
  • Posture, flexibility and muscle strength
  • Full General check up

You may bring your child for the above mentioned examinations to My Urgent Care where we have experienced providers who can do the necessary examinations and paperwork without any waiting for you. My Urgent Care has two convenient locations and we offer walk-in services with extended and weekend hours of operation. We charge the lowest fees for these services.
